Chapter 1 - 1

Pain.

Blinding, all-consuming pain.

And then—nothing.

The roar of the truck, the screech of tires, the weight of metal crushing bone—gone. Silence wrapped him like a blanket. There was no floor, no sky, no breath.

He wasn't floating. He wasn't falling.

Am I dead?

It didn't feel like a dream, but it wasn't reality either. He tried to remember his name, but it slipped through his fingers like sand. He remembered his last thoughts—he had just graduated high school, had his earbuds in, rewatching Naruto fights on the crosswalk, laughing at the irony of being a 19-year-old anime nerd with no real direction.

Guess I'm getting my isekai after all.

Something pulled him. Warm, wet, and suffocating.

And then—light.

Too much light.

His body convulsed involuntarily as cold air rushed into his lungs. He cried out without meaning to, a shrill scream piercing the air. Someone lifted him—hands soft, but firm. A voice murmured something.

"He's here. The second one."

Second?

Through blurry eyes, he saw shadows moving. A warm cloth. A tired, smiling woman. And then—

"Hirito. Hirito Uchiha," a deep voice said with authority.

The moment the word Uchiha reached his ears, something inside him snapped awake.

No. No way.

His heart—or whatever he now had in this infant form—pounded in his tiny chest.

The woman smiled down at him with kind, exhausted eyes. "Hirito…" she whispered again, brushing her thumb across his cheek. "You and your brother will be strong. I know it."

His brother?

A faint cry rang from nearby. Another baby—louder, more energetic.

The pieces began to fall into place.

The name. The woman. The voice of the father. The other baby.

Mikoto… Fugaku… and that means—

Sasuke.

He was Sasuke's twin.

Reborn into the Uchiha clan.

I'm in the Naruto world. I've been reincarnated into Naruto.

The weight of that truth crushed any remaining thoughts. His body, still fragile, demanded rest, but his mind screamed.

This was real. This was happening.

And he was exhausted already.

The first day was a blur. His muscles were too weak, his vision too blurry. Most of what he understood came from sound. Familiar voices visited the room. One in particular struck him with an odd pang of guilt and awe.

Itachi.

Five years old, but his voice was calm, respectful, quiet.

"May I see them?" Itachi asked.

"Yes," Mikoto replied gently.

Footsteps padded across the room.

Hirito forced his eyelids open just enough to glimpse a shadow lean over the crib.

He saw his older brother for the first time.

Itachi stared at the twins—first Sasuke, who gurgled and reached out with a clumsy hand. Then Hirito.

The second Itachi's eyes met his, they lingered. Just for a second.

A flicker of something crossed Itachi's face.

Hirito stared back. Silent. Still.

Please don't hate me for knowing things I shouldn't.

Itachi's fists clenched at his sides. Not in fear. Not in anger.

But in determination.

They're so small, he thought. So fragile.

He lowered his gaze. He didn't speak again that night. But in his heart, a promise bloomed.

No matter what... I will protect my younger brothers.

On the third day, Hirito began understanding just how limited he was. His neck barely turned. He couldn't crawl. His chakra—if it even responded—was buried deep within a body that didn't know how to use it.

He wanted to scream.

Instead, he studied.

Every time Mikoto picked him up, he listened to her hum. Listened to her heartbeat. Memorized the layout of the room. The creaking of the old wooden floor. The smell of fresh tea when guests visited.

Shisui visited once.

"Can't believe Fugaku-san has two sons now," he said with a smile. "They're both going to be a handful."

He gently poked Hirito's cheek. "Especially this quiet one."

Hirito blinked at him. Shisui…

He didn't know whether to laugh or cry. A character he admired, someone who would die young—standing here, alive and smiling.

Maybe this time, I can change something.

But how? He was a literal newborn. No chakra control. No mobility. No speech.

He was stuck in this helpless body… for now.

By day six, Fugaku visited more often.

He didn't hold the children often. His presence was always accompanied by silence, expectation. But he observed. Especially Hirito.

"He doesn't cry much," Fugaku said one morning. "He watches."

"Not all babies are the same," Mikoto answered with a laugh, bouncing Sasuke in her arms. "Maybe he's just shy."

"No. He's calculating."

Mikoto frowned. "He's six days old."

Fugaku didn't reply. He walked over to Hirito's crib and looked down. Their eyes met.

Fugaku's stare was like steel.

Hirito didn't look away.

Fugaku turned after a long silence. "He'll be a shinobi," he said flatly. "A strong one."

"He's just a baby."

"Even so."

The wind outside rustled the leaves of the Uchiha compound, whispering secrets through the night.

And far from the warmth of the Uchiha household, deep beneath the roots of Konoha, shadows moved.

Inside a dim, stone-walled room lit only by a flickering lantern, a masked shinobi kneeled on one knee. His voice was emotionless, cold—trained.

"A twin was born in the Uchiha clan."

Danzo Shimura, seated with arms crossed beneath his cloak, didn't flinch. His one visible eye sharpened slightly.

"Twins?" he repeated.

"Yes. Fugaku's wife, Mikoto, gave birth to two sons. One is named Sasuke. The other—Hirito."

Danzo was silent.

The agent continued. "The last time the Hyuga had twins, it produced two elite jonin. The potential threat is… concerning."

Danzo's fingers tapped slowly against his cane.

"Uchiha Fugaku is already a danger," he said coldly. "If both sons inherit the clan's eyes... and if either proves to be like have the power of Shisui..."

The agent lowered his head further. "Should we initiate contingency plans, Danzo-sama?"

Danzo stood slowly, voice low and firm. "Not yet. Accidents draw attention."

He turned to face the darkness beyond the chamber.

"But begin surveillance. Monitor the twins. The moment one shows promise… act accordingly."

The ROOT agent bowed. "Yes, Danzo-sama."

Danzo's voice echoed behind him, colder than the room itself.

"The Uchiha must not be allowed to rise unchecked."

Meanwhile, within the Hokage Tower, a report slid across the polished surface of the desk.

Minato Namikaze, the Fourth Hokage, read it with a flick of his sharp blue eyes. A smile tugged faintly at the corner of his lips.

"Fugaku had twins," he mused aloud. "Looks like the Uchiha clan just got a bit more lively."

Hiruzen Sarutobi, seated beside him in his new advisory role within the council of elders, stroked his beard thoughtfully.

"You should visit them," Hiruzen said. "Even a short appearance would go a long way in maintaining ties with the clan."

Minato nodded. "You're right. Especially with the tension we've been hearing about."

His eyes lingered on the name in the report.

Sasuke Uchiha.(Name of Hiruzen's Father).

Two boys born into one of the most powerful clans in the village.

Minato folded the report neatly.

"I'll make time to visit tomorrow."

And outside, as the village slept beneath the stars, two baby boys lay side by side.

End of the Chapter.
